Adoption Application
Want to adopt?! This form asks basic information about you, your home, and your family. Once the application has been submitted, we can help you select a dog to fit your home and lifestyle or schedule a meet and greet with one you’re already interested in.
Animal Profile
If you need to relinquish a pet, we first encourage you to review all of our notes about rehoming pets in our Community Resources page. If you do need us to courtesy post a pet, you can fill out an animal profile form here. Please also email 3+ high-quality photos of the pet and/or video to info@underdogaz.com.
Interest In Fostering
We love fosters!! This form asks basic information about you, your home, and your family, and lays out foster requirements. Once the application has been submitted, we can help you select a dog to fit your home and lifestyle or schedule a meet and greet with one you’re already interested in.
Interest In Volunteering
We are run entirely by devoted volunteers at Underdog Rescue of Arizona. If you’re interested in volunteering your time, we can absolutely use the support! We have options for in-person or at-home tasks. This form allows us to learn a bit about you and how you’re interested in helping. Once we receive, a member of our team will be in touch!
Volunteer Agreement
Volunteers play an enormous role in our ability to save more lives, serving as transport to medical appointments, assistance with administrative tasks, help with adoption intake and fosters, and support for creative tasks. This form asks basic information about you and covers important issues like confidentiality. Please fill it out completely before beginning any volunteer work with Underdog Rescue of Arizona.
